Job Responsibilities and Performance Standards:

1 Completes consumer intake based on interviews with patient/client/consumer, family members, significant others and appropriate community agencies. (Performed by all incumbents)
   ** Interviews patient/client/consumer and/or families using acceptable techniques appropriate for the individual's level of functioning in accordance with established guidelines.
   ** Contacts other health care professionals within or outside the organization to obtain consumer background information in accordance with established guidelines.
   ** Reviews patients/clients/consumers chart to obtain background information necessary to complete an assessment within established timeframe.
   ** Completes and places in patient's/client's/consumer's chart a written individual/family intake profile and updates information according to accepted standards and time frames.
   ** Complies with regulations on confidentiality of patient/client/consumer information as specified by established policy.

2 Assists other treatment team members with the identification of the patient's/client's/consumer's strengths and weaknesses and development of a treatment plan. (Performed by all incumbents)
   ** Attends and provides appropriate input in scheduled treatment team meetings.
   ** Participates in developing and reviewing appropriate treatment plans.
   ** Obtains appropriate input or information from the patient/client/consumer, or family/significant others to assist in developing appropriate goals and objectives.
   ** Accurately charts and documents patient/client/consumer progress.
   ** Provides appropriate information regarding psychological stresses and level of global functioning identified in the patient's/client's/consumer's assessment or admission notes.

3 Provides follow-up and referral services to facilitate continuity of care for patients/clients/consumers and their families. (Performed by all incumbents)
   ** Schedules follow-up appointments in a timely manner to ensure services are provided.
   ** Makes appropriate referrals for support services.
   ** Completes or monitors the completion of required forms within prescribed time frame.
   ** Meets with patient's/client's/consumer's continuity of care representative as required.
   ** Transports, or makes arrangements for transportation of, patients/clients/consumers to appointments in a timely manner.

4 Monitors patient's/client's/consumer's progress towards treatment goals. (Performed by all incumbents)
   ** Thoroughly observes patient's/client's/consumer's progress for compliance to and in obtaining treatment plan goals through one on one interviews and other contacts/interactions.
   ** Writes concise, accurate and clear observation notes in patients/clients/consumers charts as required by departmental policy.
   ** Promptly notifies appropriate professional of any unusual patient/client/consumer incident/situation.

5 Prepares and disseminates pertinent reports for/to supervisors, colleagues and other appropriate individuals. (Performed by all incumbents)
   ** Accurately prepares required periodic and ad hoc reports.
   ** Submits all reports, forms and other documentation in a timely manner.

6 Develops community resources and contacts to facilitate delivery of supported employment services. (Performed by some incumbents)
   ** Thoroughly monitors available community resources and develops new sources to meet patient/client/consumer needs.
   ** Effectively negotiates, writes and executes collaborative agreement arrangements for services.
   ** Contacts other service providers on a regular basis to maintain effective inter-agency working relationships.
   ** Serves as an effective resource person actively participating on work teams and school related special education transition planning teams.

7 Serves as liaison between patients/clients/consumers and family members, significant others, physicians, organizational components, etc. to facilitate and ensure communication, linkage and continuity of services. (Performed by some incumbents)
   ** Develops and effectively maintains a network of appropriate community contacts.
   ** Provides appropriate input in strategy or review sessions/staffings, committees and task forces to facilitate linkages and communication.
   ** Provides appropriate information and/or input in meetings pertaining to case management and case collaboration.

8 Under the guidance of a professional counselor, provides supportive counseling to patients/clients/consumers, groups or families. (Performed by some incumbents)
   ** Conducts individual sessions to provide appropriate guidance and direction pertaining to personal and social development within established guidelines.
   ** Conducts family counseling or educational sessions according to accepted methodologies.
   ** Conducts group counseling sessions using established procedures and methods to promote planned changes in patient/client/consumer support system.
   ** Provides social and psychological support to patient's/client's/consumer's and their families through listening and discussing problems and progress as necessary.
   ** Recognizes the need for patient/client/consumer crisis intervention and reacts appropriately to routine situations and/or promptly notifies person on call or immediate supervisor.

9 Assists in, or directly performs, recruitment, selection and training of agency job coach staff. (Performed by some incumbents)
   ** Assists in, or directly performs, recruiting, interviewing, initial orientation and training of job coaches in accordance with established laws, rules, regulations and policies.
   ** Meets regularly with coaches to provide guidance, advice, etc. on work related concerns/issues.
   ** Thoroughly reviews coaches' time sheets and processes within established time frames.

10 Maintains knowledge of current trends and developments in the field by reading appropriate books, journals, and other literature and attending related seminars and conferences. Applies pertinent new knowledge to performance of other responsibilities. (Performed by all incumbents)
   ** Attends internal and external educational programs and professional meetings as available for continuing professional education.
   ** Attends regular meetings of available and applicable professional organizations.
   ** Reads and evaluates professional literature on continual basis.
   ** Incorporates knowledge of pertinent new trends and developments into services. Makes recommendations for any related organizational changes.

11 Supervises or leads clerical, paraprofessional or other staff. (Performed by some incumbents)
   ** Appropriately conducts or participates in interviews of applicants for position vacancies.
   ** Provides appropriate on-the-job training to new hires and/or arranges work-related training opportunities.
   ** Assigns work to employees based on workloads, staff abilities, or other relevant factors.
   ** Structures job duties of subordinates to meet organizational goals with available resources.
   ** Consults with staff giving appropriate advice/guidance on work related issues, concerns or assignments.
   ** Approves or denies leave for other staff in accordance with established policies, procedures, rules and regulations.

12 Creates and maintains a high performance environment characterized by positive leadership and a strong team orientation. (Performed by some incumbents)
   ** Defines goals and/or required results at beginning of performance period and gains acceptance of ideas by creating a shared vision.
   ** Communicates regularly with staff on progress toward defined goals and/or required results; providing specific feedback and initiating corrective action when defined goals and/or results are not met.
   ** Confers regularly with staff to review employee relations climate, specific problem areas and actions necessary for improvement.
   ** Evaluates employees at scheduled intervals; obtains and considers all relevant information in evaluations and supports staff by giving praise and constructive criticism.
   ** Recognizes contributions and celebrates accomplishments.
   ** Motivates staff to improve quantity and quality of work performed and provides training and development opportunities as appropriate.

Entry Qualifications:

Three years of full-time (or equivalent part-time) experience in social services delivery (case management, outreach, prevention/education, etc.) OR Completion of 90 quarter hours (or its semester equivalent) of college coursework, 15 quarter hours (or its semester equivalent) of which was in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling, or an equivalent field and one year of full-time (or equivalent part-time) experience in social services delivery. OR Completion of a bachelor's degree in one of the coursework areas listed above. OR Two years of full-time (or equivalent part-time) experience as a Social Services Technician.
